To be “divorced” from reality would require a connection to it at some point. He’s never connected with reality. Always lived in his own sick world of his own making.
Truth. He's always lived in a world of privilege and no consequences. That's why he requires constant praise. His whole life people were paid to tell his mediocre ass he was special.

Got that right! Starting with his parents-bullied them and they gave in. Paid to have him go away to military school. Paid for his grades. Damaged goods from day one. Now we all are paying the price.
