Yes it's not difficult to fix this and they don't do it bc it pokes holes in the facade they'vr constructed - that they've made the world's most powerful tool that can fulfill every need. They are abdicating responsibility and acting like everything is a coincidence - typical toxic CEO behavior.
If the user-facing software you've designed doesn't have redundancies for idiots making mistakes, that's a massive design flaw.
