And here's me not even a mental health professional, and I can tell a bat-shit crazy narcissist a mile away.

I don't think you even need to know anyone with Parkinson's or Alzheimer's to realize that he's showing symptoms of one of them. He's clearly in an accelerating cognitive decline spiral. Soon they'll find an excuse to take him out of the public eye. Prob some BS like "leftist death threats".
