People are like "Leonid is being a ghoul" but actually attempting to save rural hospitals given the budget constraint is going to negatively impact urban hospitals where there is a better lives/dollar ratio on care even despite the higher overhead of being in the city
the thing about the people mad at LB is that Leonid did not create this problem, or generate false scarcity, he’s merely saying we shouldn’t gut our hospitals to support the people who voted for their own deaths. This is perfectly defensible morally and logistically!

when we get back into power, we can give healthcare to everybody even if they’d vote against it, whether they like it or not. I’m fuckin tired of defending the noble savage rural working class, who would rather see their families suffer as long as some immigrant suffers more than have both prosper.

Collective punishment is bad, and you should fight not to think that way. It may be the case that a utilitarian calculus is that it's necessary to distribute resources fairly and that will disproportionately impact the folks who explicitly claim to want that outcome, but that's not why you do it.
